Physical or Hand Therapy
Have the patient call (402) 609-1750 to schedule their initial evaluation. They can either bring their therapy script with them, or you can fax it to (402) 609‑1715. This includes splinting services as well.
With all physical or hand therapy orders, please send an updated copy of:
- patient’s photo ID
- insurance card(s)
- demographics page
Imaging
Fax the Imaging order and details to (402) 609-1820. If the patient will be using insurance and the order is for a CT or MRI, please include the completed prior authorization. We’ll call the patient back to screen them, collect insurance information, and schedule.
Detailed Requirements
- MRI Scans must be ordered WITHOUT contrast.
- CT orders can be ordered WITH and WITHOUT contrast.
- All orders must include a diagnosis code
- If no pre-authorization is required, please note that on the order.
- MRI and CT scans must be authorized under the correct NPI and TAX ID numbers for OrthoNebraska Hospital (legal name Nebraska Orthopaedic Hospital LLC):

How quickly are faxed documents reviewed?
Most faxes are reviewed within one business day.
Urgent items (post‑op concerns, STAT imaging) are handled with priority. If your fax is urgent (post‑operative concerns, infection, urgent imaging), please call us immediately after faxing to ensure rapid handling.
When are fax lines monitored?
Fax lines are monitored Monday – Friday during business hours. Items received after hours or on weekends are reviewed the next business day.
Can patients fax documents directly?
Yes. Patients may fax any requested paperwork to (402) 609-2174, including:
- FMLA/disability forms
- Personal medical records
- Release‑of‑information forms
Please include a callback number in case we need clarification.
Patients can also upload secure documents through the Cerner patient portal or through the Luma portal.
What if I sent a fax but haven’t heard back?
If you haven’t received a response within 24–48 hours, please call us.
Delays can occur due to:
- Missing pages
- Illegible scans
- Missing patient identifiers
- Provider‑required review
We can quickly confirm whether your fax was received.
Do you accept digital or secure electronic submissions?
Yes. Depending on your situation, we may support:
- Encrypted HIPAA‑secure email (for approved partners)
- Direct EHR‑to‑EHR exchange
- Patient portal uploads
