What Is Turf Toe?
Turf toe is a sprain of the main joint of the big toe, an injury that occurs when the toe is forced into hyperextension. Think of a sprinter’s start position or the push-off phase in many sports. While the name reference football players on artificial turf, this injury can happen to anyone.

What Causes Turf Toe?
Turf toe typically occurs when the front of the foot is fixed to the ground while the heel lifts and an external force pushes the big toe upward. It’s common in athletes who play on hard, artificial surfaces with flexible shoes that allow excessive motion in the forefoot.
Although most common in football, turf toe also affects dancers, gymnasts, soccer players and runners — anyone who repeatedly pushes off the ground with force.
Turf Toe Symptoms
Turf toe symptoms often appear immediately after injury and can get worse with activity. You may notice:
- Swelling and bruising around the big toe
- Pain when bending or moving the toe
- Tenderness on the underside of the joint
- Difficulty pushing off while walking or running
The severity of the injury is classified into three grades, from mild swelling to a complete tear with instability of the big toe.
How Is Turf Toe Treated?
Most turf toe injuries heal without surgery. Treatment depends on severity but could include:
- RICE: Rest, ice, compression and elevation
- Bracing or taping to limit motion and protect the joint
- Footwear modifications like stiff-soled shoes or custom orthotics to relieve pressure on the toe
- A walking boot for moderate to severe injuries to allow healing
Over-the-counter anti-inflammatory medications (like ibuprofen or naproxen) can help reduce discomfort during recovery.
Surgical treatment is rare for turf toe but could be recommended if there is bone/cartilage damage or a complete tear.
Depending on the severity of the injury, it could take up to a year to recover completely, but mild cases of turf toe are usually healed within a couple months.
