Please plan to arrive 15 minutes prior to your appointment time, and bring the following materials with you to simplify your check-in process.
- Picture ID (driver’s license, state-issued ID card, military ID, or passport)
- If the patient is a minor, a parent/guardian ID.
- Current Insurance Card(s)
- Supplementary Records or Imagery, if these have not previously been sent to our office or if you have copies from another medical center or physician’s office.

If you go to urgent care and can barely walk, the mri should be entered in as "urgent" so you can get it scheduled quicker.

I think it is really sad and says a lot about how you really care for your patients when they can not get in to see the provider for 2 weeks and they have to go to the urgent care . All that was wanted was a shot and the doctor and the pa were both too busy to do that, this was not the first or even the second time to get this shot. I have been told that these become less effective the more times you get them but sounds like they just want to schedule them every 3 months rather than get you in when it is needed. Want the parented income rather than patient needs?quite disappointed with patient Concern
