- If the patient is a minor, a parent/guardian ID.
- Current Insurance Card(s)
- Supplementary Records or Imagery, if these have not previously been sent to our office or if you have copies from another medical center or physician’s office.

First I didn't know that Ortho Urgent Care existed, how great is that? The staff was awesome from the nurse to the X Ray tech to the PA. I left knowing a whole lot more about my condition and how to treat it. Two days later and I can't believe how bad I let it get before getting help. I'm almost at 100% back on my feet, thanks!

I'm grateful I received great care having to go to urgent care since I couldn't get in for a knee appointment in the time frame I needed to. Everyone was so kind, so professional and so welcoming. I'm a repeat client and will always use this facility!
