- Picture ID (driver’s license, state-issued ID card, military ID, or passport)
- If the patient is a minor, a parent/guardian ID.
- Current Insurance Card(s)
- Supplementary Records or Imagery, if these have not previously been sent to our office or if you have copies from another medical center or physician’s office.

Just thanks for seeing me without appointment

I wanted a radiology report to share with my PT but a radiologist does not read X-rays from urgent care. I would prefer a radiologist interpret my X-ray rather than a nurse practitioner. There is very little information on my chart in the portal should I need follow up. Please consider having a radiologist interpret X-rays and submit a report. Thank you
