The Form was for a New Patient. My visit was a post-surgery follow-up! If the Form offered a check off box option:(Follow-up) it would be less encumbering to answer questions as if a new patient!

Dr. Jassim was excellent. He answered all my questions and was very upfront. I only waited about five minutes, which is fantastic.
