Where to Go

  • Drive to the Oakview Medical Building at 144th and West Center Streets. As our largest location, we have several different offices, and the exact location of your appointment will be in your appointment reminder. Suites 110, 205 and 212 are all located within the Oakview Medical Building.

If your appointment is in Suite 110:

  • Park near and enter the building through the East B Entrance.
  • Proceed through the lobby and Suite 110 is the first door on the right, just past the elevators.

  • Park near and enter the building through the West B Entrance.
  • Proceed past the physical therapy lobby and front desk and Suite 212 is on the left.

  • Park near and enter the building through the West B Entrance.
  • Proceed past the physical therapy lobby and front desk, then turn right. Suite 205 will be on the left.

What to Bring With You

Bring the following materials with you to simplify your check-in process.

  • Picture ID (driver’s license, state-issued ID card, military ID, or passport)
    • If the patient is a minor, a parent/guardian ID.
  • Current Insurance Card(s)

I felt that PA was not 100% listening to me. Dr. Hahn and Nurse Carol were great.

Dr. Missed the spot in the right knee. Hurt bad then he moved the needle over and was OK. By evening I couldn't walk or put any weight on it. Cortisone flair no doubt. I'm walking with a cane today. Very painful
